2008-2009 Sports Physicals
Tuesday, August 12th from 5:30-7:00pm in the Erie High School Commons
All Erie CUSD #1 student athletes (grades 6, 7, 8 & 10, 11, 12) may participate in the August physicals. Ninth graders must have a regular physical at their physician's office that will serve as a sports physical.
*Please note that sports physicals are good for 1 year and must be completed and handed in by the first day of practice.
Forms may be picked up at the MS or HS offices the last week of school or on the day of the physicals, August 12, 2008. Parents must sign the permission slip as well as the parent's section of the physical form prior to the physical, or accompany the student athlete to the physical. There will be a cost of $5.00 and there will be no scheduled appointments.
