Who is required to have a physical?
-First Year Preschool Students
-Kindergarten Students
-Sixth Grade Students (starting fall 2009)
-Ninth Grade Students
-Students New to the District
When do physicals need to be completed?
-Students must have a physical prior to the first day of school.
-New students have 10 days to complete their Illinois physical
What forms are required to be given to the school nurses?
-A completed Illinois Physical Exam Form (An Illinois Physical Exam Form may be printed via this link: Default Hyperlink Text)
-Proof of up-to-date immunizations
-Kindergarten and Preschool students must also show proof of having a lead screening or test.
-Birth certificate
