State of Illinois Dental Exam Requirements

Illinois State Dental Exam Requirements

Who is required to have a dental exam?  All Kindergarten, second and sixth grade students.

(School nurses send dental forms out in the spring to future kindergarten students and current first and fifth grade students, so parents have  over a year to complete these forms)

When is the deadline for dental exams?  All completed dental forms must be turned in by May 1st, 2009.

Most dental offices will have this form on hand or a copy of this form can be printed via this link: http://www.idph.state.il.us/HealthWellness/oralhlth/DentalExamProof.pdf  You may also pick up a form at either the elementary or middle school nurse's offices.

Please note if this is a financial burden for your family, there is a Dental Clinic in the Whiteside County Health Department that utilizes a sliding fee scale. Information about their program can be obtained by calling the Whiteside County Health Department @ (815)772-7411.  Please note that their dental clinic is very busy, and you will need to make an appointment early!  Thank you for helping us comply with this state requirement, and should you have any questions concerning this matter please feel free to contact the school nurses @ 659-2239 ext. 2525
